Heath Ledger and the makeup artists decided that this version of the Joker doesn't have bleached skin; he paints his face himself. To ground this in reality, they added a subtle costume/makeup detail: whenever the Joker is not wearing gloves, you can clearly see white greasepaint and red lipstick heavily smeared on his fingertips and under his nails from constantly applying and touching up his own chaotic disguise.
