The metal lid-locks used on Malcolm McDowell's eyes were genuine medical instruments. Despite the doctor in the scene being a real physician repeatedly applying eye drops, the clamps actually slipped during the take and severely scratched McDowell's cornea. His agonized screams in the scene are completely real, and the injury temporarily blinded him.
